Agency for Healthcare Research and Quality: Medical Errors and Patient Safety The Agency for Healthcare Research and Quality (AHRQ) research provides evidence-based information on health care outcomes; quality; and cost, use, and access. AHRQ was formerly known as the Agency for Health Care Policy and Research. One area of AHRQ research addresses measures and strategies to improve patient safety. |

Council on Graduate Medical Education: Collaborative Education to Ensure Patient Safety This report describes the results of a joint meeting between national advisory councils in Medicine and Nursing on collaboration between physicians and nurses to enhance patient safety. It was carried out by the Council on Graduate Medical Education (COGME) and the National Advisory Council on Nurse Education and Practice (NACNEP).
