  • > Agency for Healthcare Research and Quality:
    Medical Errors and Patient Safety
    The Agency for Healthcare Research and Quality (AHRQ) research provides evidence-based information on health care outcomes; quality; and cost, use, and access. AHRQ was formerly known as the Agency for Health Care Policy and Research. One area of AHRQ research addresses measures and strategies to improve patient safety.

    ASHP has expanded its focus on patient safety in the medication-use process with the creation of the Resource Center on patient safety. The site includes information about Center programs, links to news items and reports, error reporting programs, practice resource areas, etc.
     
    > Annals of Internal Medicine
    The Annals is publishes occasional "quality grand rounds, a case-based series on medical errors and patient safety." The first one appeared in June 2002. The following links to full-text will work for UCSD users and others with licensed access to the Annals online.

    > Council on Graduate Medical Education:
    Collaborative Education to Ensure Patient Safety
    This report describes the results of a joint meeting between national advisory councils in Medicine and Nursing on collaboration between physicians and nurses to enhance patient safety. It was carried out by the Council on Graduate Medical Education (COGME) and the National Advisory Council on Nurse Education and Practice (NACNEP). The site includes a summary of the report, a PDF version of the full 150 page report, and other Council on Graduate Medical Education resources.
     
    > Institute for Safe Medical Practices
    ISMP is a nonprofit organization that works with healthcare practitioners and institutions, regulatory agencies, professional organizations and the pharmaceutical industry to provide education about adverse drug events and their prevention. Site includes medication safety alerts and error reporting forms.

    The mission of the Institute of Medicine is to advance and disseminate scientific knowledge to improve human health. The Institute provides objective, timely, authoritative information and advice concerning health and science policy to government, the corporate sector, the professions and the public.

    Composed of more than 170 public and private organizations that provide health care benefits, The Leapfrog Group works to improve hospital systems that could break down and harm patients. Representing approximately 32 million health care consumers in all 50 states, Leapfrog provides information and solutions for consumers and health care providers.
     
    > National Guideline Clearinghouse
    A public resource for evidence-based clinical practice guidelines. NGC is sponsored by the Agency for Healthcare Research and Quality (formerly the Agency for Health Care Policy and Research) in partnership with the American Medical Association and the American Association of Health Plans.
     
    > National Patient Safety Foundation
    The mission of the National Patient Safety Foundation (NPSF) is to improve patient safety in the delivery of health care. The Foundation offers educational programs and sponsors research.

    Information for Health Professionals
    FDA's mission is to promote and protect the public health by helping safe and effective medical products reach the market in a timely way, and monitoring products for continued safety after they are in use. The FDA Website has resources for health professionals about regulated products and agency policies of interest to the medical community. The site also supports online reporting of problems with medical products through the MedWatch program.
